2008 Winter Academy League

The following forms will need to be completed and returned for registration for Valrico Soccer Club’s Winter Academy:

HCPRD Informed Consent

HCPRD Medical Release (MUST be Notarized)

These three forms must be completed and mailed to the address at the bottom of this page. Please include a copy of the player’s Birth Certificate as well as the Registration fees of $90.00 per player.

FORMS MUST BE POSTMARKED BY OCTOBER 15, 2008

Due to the number of registrations received, you may want to send a note to the e-mail address below verifying that the Registration Forms have been received and are complete. If forms are not complete your player will not be considered as registered.
